90 Minute Early Release Information
2012-2013

Dear Parents/Guardians,

Attached you will find the registration and contract forms needed to enroll your child / children in the 90 min. Early Release Care Program.  Please return the completed registration form and contract, along with the $100.00 tuition fee, (per child), 10% discount for families with 2 or more children enrolled.  Your child will not be able to attend the program if payment is not received prior to the first day of school for the 12-13 school year.
Checks should be made payable to Pentucket BASP.

Forms should be delivered by hand, or mailed to:

Please remember to call the BASP if your child / children will not be attending or will be late arriving to  the BASP on their scheduled day due to absence, going to a friends house, Brownies, Girl Scouts, Boy Scouts, PTO activity etc, etc, etc……  Failure to contact the BASP BY PHONE with changes will result in a $10 fee.
